Post-traumatic arthropathy

Post-traumatic arthropathy is a form of degenerative arthritis that develops as a late consequence of a previous joint injury, such as an intra-articular fracture, dislocation, ligament tear, or severe damage to the articular cartilage. The initial injury disrupts the normal anatomy and biomechanics of the joint, gradually leading to uneven distribution of mechanical loads across the articular surfaces. This process accelerates cartilage deterioration and results in premature degenerative changes, even in younger patients without other predisposing factors.

The onset of post-traumatic arthropathy varies considerably, ranging from several months to many years after the initial injury, depending on the severity of the damage, the quality of the initial treatment and rehabilitation, and the presence of persistent joint instability or dysfunction.

Symptoms are similar to those of primary osteoarthritis and include pain, stiffness, swelling, and progressive limitation of joint motion. It commonly affects weight-bearing joints, such as the knee, ankle, and hip, although it may occur in any previously injured joint.

Diagnosis is based on the history of previous trauma combined with clinical examination and imaging studies. Treatment ranges from conservative measures, including physical therapy, activity modification, and medication, to surgical procedures such as joint reconstruction or arthroplasty in advanced cases with significant pain and functional impairment.
