Obstructive Osteoarthritis

Obstructive osteoarthritis is a form of degenerative joint disease in which progressive deterioration of the articular cartilage is accompanied by prominent osteophyte (bone spur) formation and fibrotic thickening of the joint capsule. These changes can mechanically restrict or even completely block the normal movement of the affected joint. Unlike milder forms of osteoarthritis, where pain during movement is the predominant symptom, obstructive osteoarthritis is characterized primarily by mechanical obstruction caused by osteophytes or loose intra-articular bodies that interfere with the smooth motion of the joint surfaces. The condition most commonly affects weight-bearing joints, particularly the knee and hip, and is associated with risk factors such as aging, obesity, previous joint injuries, and chronic mechanical overload. Patients typically experience joint stiffness, a reduced range of motion, and episodes of locking or catching. Diagnosis is established through clinical evaluation and confirmed with imaging studies, including X-rays or computed tomography (CT) scans, which demonstrate the structural changes responsible for the mechanical limitation. Treatment depends on the severity of the condition and ranges from conservative management—including physical therapy, lifestyle modification, and medication—to surgical intervention, such as arthroscopic debridement or total joint replacement, in advanced cases where pain and functional impairment significantly affect quality of life.
