Cervical micro-instability

Cervical microinstability refers to a mild, functional disorder of stability between adjacent cervical vertebrae, which does not reach the level of classic, radiologically documented instability, but manifests as impaired movement control due to ligamentous laxity or reduced neuromuscular stabilization of the region. It is usually caused by mild, repetitive microtrauma of the ligamentous structures, as may occur after a mild whiplash injury, by weakness of the deep stabilizing muscles of the neck, as well as by early degenerative changes of the intervertebral discs, which reduce the passive stability of the spinal segment. Unlike severe cervical instability, microinstability is often not clearly visible on standard radiological evaluation but presents clinically with recurrent episodes of neck pain, a sensation of “looseness” or insecurity during movement, muscle fatigue, and occasionally mild associated dizziness, particularly after prolonged mechanical stress of the region. Assessment is primarily based on clinical examination, with emphasis on evaluating neuromuscular function and cervical proprioception, while imaging studies are mainly used to exclude more severe structural instability or other pathology. Treatment focuses on a physiotherapy program with targeted strengthening exercises of the deep cervical muscles and proprioceptive retraining, aiming to improve the dynamic control of cervical stability.
