Load asymmetry

Load asymmetry refers to the uneven distribution of body weight or mechanical forces between the two sides of the body, or between different parts of a joint, during standing, walking, or physical activity. It may result from anatomical abnormalities, such as leg length discrepancy, scoliosis, or limb deformities, as well as from functional factors, including muscle weakness or pain in one limb that causes the patient to unconsciously reduce weight-bearing on the affected side. Poor postural habits and altered movement patterns may also contribute to the development of load asymmetry.

Prolonged asymmetric loading disrupts normal joint biomechanics, leading to excessive stress on specific anatomical structures. This may accelerate cartilage degeneration, contribute to chronic pain, and promote the development of degenerative joint changes. Load asymmetry is frequently observed in patients with knee or hip osteoarthritis, where pain alters gait patterns through compensatory mechanisms, as well as in athletes exposed to uneven training loads between limbs.

Assessment of load asymmetry is performed through clinical evaluation of posture and gait, while specialized centers may use advanced gait analysis systems or pressure measurement platforms to quantify differences in loading patterns.

Treatment focuses on restoring balance between the two sides of the body through targeted strengthening exercises, postural correction, movement retraining, and, when necessary, the use of orthotic devices to improve alignment and optimize mechanical function.
